Course content
Lectures: 1. Conception of specialized care, department and organization. Hygienic regime of Anesthesiologic-resuscitation department and intensive care units, prevention of nosocomial infections, MRSA. Rules of antibiothic treatment. 2. Ethic questions of resuscitation care. Dying and death at Anesthesiologic-resuscitation department. Diagnostics of brain death, care of a cadaver, ethic aspects. Complex care of a patient put into a transplantation programme. 3. Invasive and non-invasive monitoring in intensive care. Monitoring of haemodynamics. Instrumental equipment in intensive care. 4. Substitutive intravenous solutions. Blood derivatives, rules of effective haemotherapy. Nutrition support in intensive care. 5. Pharmaceuticals in intensive care. Analgosedation in intensive care. Scoring schemes in intensive care. 6. Urgent respiratory failure, ALI, ARDS. Securing of airways in intensive care. Bases of artificial pulmonary ventilation. Care of a patient at APV. 7. Intoxication. Specifics of care of a patient after drug, mushroom, CO intoxication. 8. Basics of anesthesia and analgesia, pain treatment - 1. part - general anesthesia, securing od airways during anesthesia, complications of global anesthesia, monitoring of the patient during anesthesia, preparation of the patient before anesthesia, anesthesia risk assesment - ASA classification. 9. Basics of anesthesia and analgesia, pain treatment - 2. part - local anesthesia, bases of pharmacology of drugs used in anesthesiology, urgent treatment and chronic pain. 10. Urgent resuscitation (ERC Guidelines 2010) and complex care of patients after resuscitation, therapeutic hypothermia. Electroimpulsotherapy. Seminars: 1. Practice of chosen nursing techniques used at Anesthesiologic-resuscitation department and intensive care units. 2. Complex care of a patient in intensive care, prevention of complications caused by long-term bed stay including the care of patient's psyche. Immobilization syndrome. Rules of rehabilitation and position adjusting of patients in intensive care. 3. Complex care of an unconscious patient. Specifics of care of a patient in apallic state in intensive care. Bases of basal stimulation. 4. Specifics of intensive care of children patients including psychological approach. Complex intensive care of a patient after protrahovaný surgical intervention. Specifics of care of a patient with coagulation disorders - DIC. 5. Specifics of care of a patient with serious disorders of viscera, metabolism disorders (glycaemia disorders), water and electrolytic economy disorders and acidobasic acid-base balance disorders. 6. Complex care of a patient with bleeding into gastrointestinal tract, with acute necrotizing pancreatits. Fulminant liver failure and options of its therapy (instrumental support of liver function - Prometheus, MARS). Pathophysiology of the gastrointestinal tract, reaction of gastrointestinal tract, to critical organism threat, specifics of care of these patients. 7. Complex care of a patient with acute liver failure. Hepatorenal syndrome. Acute tubular necrosis. Pathophysiology of urinary system, liver function failures with impact on critical organism condition, care of patients with continual elimination methods (CVVH, CVVHD, CVVHDF, plasmapheresis, SCUF) - principles, anticoagulation, monitoring. 8. Specifics of care of a patient with life-threatening trauma and polytrauma. Specifics of care of a patient with serious burns. 9. Acute cardiovascular diseases, investigative methods of heart and blood circulation, care of critically diseased in cardiology. 10. Shock - pathophysiology, types and therapy. SIRS, sepsis, MODS, MOF. Specifics of care of patients in critical state, multi-organ failure.

learning outcomes
A graduate of a course will theoretically and practically master treatment of patients with failing or failure of elemental vital functions, their support or substitution. He/she will be also familiar with basic procedures and techniques of global and local anesthesia and analgesia.
